Referring Employees to an SAP

If any employee tests positive on a random, suspicion, or other drug/alcohol test, please call us at 1-800-937-7770. Please provide HBA with the employee’s name, address, home and work phone numbers, social security number and birthdate. We need to know what the employee tested positive for, the date of the test, current employment status, and if this is a DOT case, the name of the Designated Employer Representative and who is responsible for the charges associated with this service.
