CLAIMS INFORMATION
Providers and facilities are requested to submit all claims for treatment services directly to HBA.
A claims examiner will review the claim against the treatment authorization. EAP claims are paid by
HBA directly to the provider. Managed Care claims will be forwarded to the Third Party Administrator
or other claims payer with an authorization form indicating authorized treatment and charges within
three working days of the “clean” claim’s receipt by HBA.
All claims should be submitted on standard medical claims forms. Call HBA 1-707-747-0117,
