HBA has an ongoing need for qualified Substance Abuse Professionals as the third party administrator of SAP services for Federal, State and Municipal government agencies and private trucking companies and other employers of “safety-sensitive” employees. HBA refers employees for SAP evaluations nationwide.

Substance Abuse Professionals who meet the federal DOT qualification standards should submit an application for inclusion in HBA’s provider panel under Provider Application, and indicate SAP credentials.

As a provider of Substance Abuse Services, particularly for DOT regulated cases, it is critical that you follow the procedures outlined on the SAP Procedures Information sheet. Necessary instructions and forms can be downloaded and printed below. When submitting SAP clinical information, please fax items to HBA at (707) 747-6646, as e-mail is not a confidential means of transmittal.
